WellBridge Community Care Program

WellBridge Community Care Program provides comprehensive mental health and wellbeing services to seniors in skilled nursing facilities and assisted living communities. Our fully integrated team of psychiatrists, psychologists, nurse practitioners, licensed social workers and educators is uniquely qualified to address the mental health and wellbeing of seniors.

Our mission is to provide compassionate, sophisticated and innovative care that enhances the lives of patients and their caregivers and improves the quality and performance of senior care facilities.

As seniors age, the staffs at skilled nursing facilities and assisted living communities have expertise to help with physical healthcare needs from rehabilitation to wellness.  However, with one-third to one-half of residents in long-term care facilities having a diagnosable mental health condition, not addressing those issues can impact the quality of life for the residents and their caregivers as well as the quality and performance of the facility.

Mental health and wellbeing issues facing seniors vary widely, and those suffering often exhibit any or all of the following:

Confidence Through Education

Educational in-service programs help your staff build skills, confidence, and effectiveness in caring for residents with behavioral health conditions.

Mental Health Assessments & Testing

Mental health assessments, including memory evaluations and neurocognitive tests, guide the development of individualized treatment plans using evidence-based practices.

Behavioral and Pharmacological Interventions

Our multidisciplinary clinical teams integrate pharmacological and behavioral interventions, emphasizing non-pharmacological treatments whenever possible.

Treatment

Our team of psychiatrists, nurses and social workers work collaboratively with your clinical staff to create a individualized treatment plan for every patient. Our plans are proactive and prioritize non-medicinal interventions such as individual or group therapies, behavioral monitoring and outpatient treatment when appropriate.

Medication Management

Our teams of psychiatrists and nurses are experts in managing behavioral health medications. We carefully evaluate the patient’s condition and treatment options as part of a comprehensive treatment plan focused on non-pharmacological treatments first.  When medications are needed, our teams carefully manage and supervise their use.  We also focus on Gradual Dose Reductions to help meet or exceed compliance of F-tag 329 guidelines.

24/7 Support

We are available 24/7 to respond to your behavioral health needs. Our goal is to treat patients in place at your facility, but in the event a transfer to an acute care or intensive outpatient care setting is required, we can facilitate that admission.

EDUCATION

Education Creates Confidence

Our Community Care Program gives your staff clarity and confidence identifying and responding to potential mental health behaviors.   We deliver proactive, scheduled, educational in-service training programs covering the behavioral health

Without effective behavioral health training, even the most talented clinical staff and social workers can be challenged by behavioral health issues, leading to frustration, burnout and turnover.    Moreover, without training, the reactions to potential mental health conditions can be inconsistent, making resident care more complicated and leading to unnecessary challenges for the patients, their families and your staff.

Educational in-service programs are designed to help your staff build skills, confidence, and effectiveness for elderly mental health care.  Our educational in-service programs help to:

  • Provide access to mental health professionals
  • Equip staff with training and education regarding the treatment of residents with behavioral health needs
  • Create onsite reference materials
  • Build in house staff skills and confidence
